Job summary

TURN is seeking a dedicated behavioral health professional to provide case management, crisis intervention, and comprehensive support across medical, educational, social, and housing resources. The role involves coordinating referrals, advocating for clients, and ensuring whole-person care in collaboration with ACT and ICM teams.

The ideal candidate holds a Master’s degree (or equivalent), experience in social services, and familiarity with mental health or substance use services; bilingual

Qualifications

  • Master's degree or bachelor's degree with four years of experience in social services.
  • SUD CM certification from a recognized board is preferred and registration required; TB test and CPR/First Aid certification upon hire.
  • Housing experience: two years with an AA in social service or related field or high school diploma with 5 years of experience.
  • CA driver's license and current auto insurance required.
  • Experience with mental health and/or substance use disorder services preferred.
  • Cultural competency understood; bilingual preferred.
  • Experience with Microsoft Office and EHR software preferred.

Responsibilities

  • Provide case management with crisis intervention, discharge planning, group and individual work, and counseling.
  • Complete accurate documentation and meet minimum billable requirements if contractually required.
  • Facilitate placement and transportation as needed.
  • Shift coverage including part of the on-call team.
  • Provide field-based services as required by program and client need.
  • Maintain confidentiality of client care and participate in program meetings.
  • Collaborate with ACT team members and Program Manager for smooth transitions between ACT and ICM levels of care.

Job Summary

Recommend and assist access to medical, educational, social, vocational, housing, and more to support program goals. Initiate referrals to next level care, coordinate with physical health providers for whole-person care, and advocate in the best interest of clients.

Responsibilities

  • Provide case management with crisis intervention, discharge plan, group & individual work, mental health/substance use disorder counseling, screening, and assist clients in meeting basic needs and symptom management.
  • Complete accurate and timely documentation, meet minimum billable requirements for direct client services if contractually required.
  • Facilitate placement including pre-placement visits, and transportation as needed.
  • Shift coverage including part of the on-call team.
  • Provide field-based services as required by program and client need.
  • Maintain confidentiality of client’s care.
  • Participate in scheduled program meetings.
  • Other relevant duties as assigned.
  • Collaborate closely with ACT team members and Program Manager to support smooth transitions between ACT and ICM levels of care.
Qualifications
  • Master's degree; or bachelor's degree with four years of experience in social services.
  • For SUD CM a certification from recognized board is preferred and registration required and a negative tuberculosis test (TB) is conducted under licensed medical supervision within 45 working days of hire or within 5 working days of employment and valid cardiopulmonary resuscitation (CPR) and first aid certification upon hire or within 90-days of employment.
  • For Housing two years of experience and an AA degree in social service or related field or High School Diploma and 5 years’ experience.
  • Valid CA driver’s license and current auto insurance per contract requirements required.
  • Experience providing mental health and/or substance use disorder services preferred.
  • Cultural competency understanding required.
  • Bilingual preferred.
  • Knowledge of Microsoft Office and EHR software preferred.
