TURN is seeking a dedicated Case Manager in San Diego to coordinate access to medical, educational, social, vocational, housing, and other services. You will initiate referrals, advocate for clients, and work with physical health providers to deliver whole-person care.
The role emphasizes crisis intervention, discharge planning, collaborative teamwork, and maintaining client confidentiality while supporting program goals and ensuring timely documentation.
