Job summary

Valley Community Services Board is seeking a Licensed Practical Nurse to provide nursing services in two residential group homes serving individuals with intellectual and developmental disabilities in Staunton, Virginia. You will work with physicians and Direct Support Professionals to ensure health needs are met in a coordinated, person-centered care setting.

Qualifications

• Current, valid certification as a Licensed Practical Nurse (LPN) in the Commonwealth of Virginia
• Valid Virginia driver's license with at least zero points.

Responsibilities

• Provide nursing services in consultation with psychiatrists, primary care physicians, Direct Support Professionals, and other members of the support team.
• Assess individuals' physical health status at least quarterly and identify changes or concerns requiring follow-up.
• Review daily progress notes and documentation related to medications, medical appointments, and medical procedures.
• Oversee medication administration and the inventory, storage, acquisition, and management of medications, medical supplies, and equipment.
• Provide medication administration training, guidance, and ongoing support to ICF team members.
• Work collaboratively with residential staff, healthcare providers, and other partners to support individuals' ongoing healthcare needs.
• Develop and maintain effective working relationships with team members and external healthcare partners.

Valley Community Services Board is seeking a Licensed Practical Nurse (LPN) to provide nursing services to individuals with intellectual and developmental disabilities living in two residential group homes. In this role, you'll help support the health and well-being of the individuals we serve while working closely with physicians, Direct Support Professionals, and other members of the support team.

What You'll Do

Minimum Qualifications

The most competitive candidates will have one or more of the following additional qualifications.

• Previous experience serving individuals with intellectual or developmental disabilities.
• Previous experience working in an intermediate or long-term care facility.
