Job summary

CRi is seeking an experienced Licensed Practical Nurse (LPN) to provide skilled nursing care to individuals with ID/DD in their homes in the Vienna, VA area. The LPN will supervise DSPs, work with the interdisciplinary team, and ensure care plans comply with physician orders and regulations.

Candidates must hold a Virginia license or compact state license, have at least 2 years of nursing experience, and possess a valid driver's license.

Qualifications

• Virginia LPN license or Nursing Licensure Compact eligibility.
• Minimum 2 years of current nursing experience.
• Valid driver’s license.
• Experience with ID/DD population preferred.

Responsibilities

• Provide skilled nursing care to individuals in their homes.
• Supervise and train Direct Support Professionals in healthcare needs.
• Collaborate with interdisciplinary team in planning and service provision.
• Coordinate services with family members.
• Perform nursing assessments and emergency care as needed.
• Document care using the Electronic Medical Record (EMR).

Licensed Practical Nurse (LPN) – Vienna, VA

CRi is a non‑profit agency that provides quality services to empower individuals with a wide range of disabilities to live independently and with the highest possible quality of life. Our main office is located in Chantilly, VA and we provide services throughout the DMV area.

We are looking for an experienced LPN to provide quality skilled nursing care to individuals with intellectual/developmental disabilities who live in their own homes, in accordance with the physician’s orders and state/federal regulations. The LPN will provide supervision and training to the Direct Support Professionals (DSP) in the healthcare needs of the individuals, work alongside the interdisciplinary team in planning and service provision, coordinate services with family members, and be responsible for performing nursing assessments and emergency care as needed.

Requirements

• Licensed Practical Nurse in Virginia or in a participating state in the Nurse Licensure Compact.
• At least 2 years of current nursing experience.
• Valid driver’s license.

Preferred Qualifications

• Experience with ID/DD population.

Other Qualifications

• Ability to perform nursing services for individuals with intellectual and other developmental disabilities.
• Demonstrated comprehensive nursing skills and skills in the use of standard medical equipment.
• Written and verbal communication skills for documentation and communication with families, community providers and team members.
• Ability to work independently to complete reports and document pertinent information in medical records using the Electronic Medical Record.
• Ability to perform the essential physical requirements of the position, which may include standing, walking, running, and lifting non‑ambulatory individuals.
