Kaleidoscope Family Solutions ABA, Inc. is hiring an IBHS Case Manager in Bryn Mawr, PA to oversee health and proactive relationships with children, families, and schools served by the IBHS program.

You will coordinate services, monitor outcomes, maintain the census, and ensure eligibility with CBH, collaborating with BCBA/LBS teams and clinicians.

Bachelor's degree required and at least three years of related experience; strong organization and transportation are essential.

Qualifikationen

• Bachelor's degree in social work, social science, nursing, or BA from an accredited institution.
• Minimum of three years' case management or related mental health experience with children and families.
• Reliable transportation and knowledge of IBHS regulations is a plus.

• Assess service needs based on physician/psychologist evaluation via IBHS intake.
• Identify and coordinate services including IBHS packets and onboarding children.
• Ensure CBH eligibility for IBHS participation.
• Coordinate with clinical and recruiting teams and CBH/schools.
• Maintain weekly follow-up meetings with client and parent.
• Document care coordination in the electronic health record.
• Support billing documentation and weekly service hours.
