Job summary

Qualifications

• MSW degree from CSWE-accredited program.
• Current full, active LICSW licensure in any state.

Responsibilities

• Provide brief cognitive/behavioral interventions in primary care under Air Force IBHC model (4 appointments max).
• Manage a daily patient list (~8-14 encounters).
• Collaborate with PCMH team and consult with primary care managers as needed.
• Document care and recommendations in the EMR within standard timelines.
