Qualifications

• Master’s degree in Social Work (MSW) from a CSWE-accredited program.
• Active, unrestricted license to practice as a Clinical Social Worker.
• Current BLS Healthcare Provider certification or ability to obtain prior to start date.

Responsibilities

• Provide evidence-based individual counseling in a primary care/behavioral health setting.
• Serve as IBHC/BHCF in patient-centered medical home environments.
• Collaborate with PCM and care teams to manage behavioral health needs of enrolled population.

Education

MSW (CSWE-accredited)

Active, unrestricted LCSW license

AHA/ARC BLS Healthcare Provider certification
