Job summary

Old Colony Elder Services (OCES) is seeking a dedicated Geriatric Support Service Coordinator (GSSC) to work in our Senior Care Options department to help determine eligibility and ensure the provision of appropriate services to eligible members.

This role offers a 35-hour work week with a hybrid work environment (home, office, and home visits), opportunities for professional development, and generous benefits including PTO, company paid LTD and life insurances, and a 401K plan.

Qualifications

• Requires at least two years in human services, elder population preferred.
• Bachelor's degree in social work or related field preferred; licensure if appropriate.
• Strong written and verbal communication; adaptable to program changes.

Responsibilities

• Assess health and functional status of members and develop care plans.
• Promote independent functioning in least restrictive environment.
• Maintain regular contact, including home visits, to monitor progress.
• Maintain accurate documentation.

We are seeking a dedicated, bilingual preferred Geriatric Support Service Coordinator (GSSC) to work in our Senior Care Options (SCO) department to help determine eligibility and ensure the provision of appropriate services to eligible members.

What You'll Do

• Promote the independent functioning of members and provide services in the most appropriate, lease restrictive environment
• Perform initial/ongoing assessments of the health and functional status of members and develop individualized care plans
• Develop rapport with consumers to facilitate coordination, monitoring, and adjusting care plans as needed
• Provide regular contact with consumers, including home visits, to ensure progress towards established goals
• Maintain accurate documentation

What You'll Bring

• Your passion for helping others and desire to make a difference.
• At least two years of experience in the human services field, preferably with the elder population
• Bachelor's degree in social work or related field highly preferred; current licensure if appropriate
• Adaptability to be able to change as the needs of consumers and/or program change
• Excellent written and verbal skills.
• Excellent computer skills.
• Proficient with Microsoft Office Suite or related software.
• Bilingual highly preferred
