Job summary

Integrated Services for Behavioral Health (ISBH) is seeking an I-FAST Community Behavioral Health Worker to support youth and families in Fairfield County, OH. You will provide in-home and community interventions, coordinate with schools and courts, and help families achieve behavioral health goals.

Minimum requirements include a high school diploma or GED and 2–3 years in healthcare or behavioral health. A valid driver's license and ability to travel are essential; comprehensive benefits are

Qualifications

• High school diploma or GED required.
• Bachelor's degree in healthcare, behavioral health or related field or 3+ years of relevant experience.
• 2-3 years direct experience in healthcare or behavioral health preferred.
• Experience with multisystems such as Courts, Child Protective Services, Schools, OhioRise, Family and Children First Council.
• Ability to bill Medicaid without restrictions.
• Valid driver's license and ability to operate a motor vehicle; appropriate auto insurance.
• Strong communication and organizational skills; ability to maintain confidentiality.
• Willing to participate in cross-systems team-building activities.

Responsibilities

• Engage individuals and families in the home and community to identify strengths, barriers, and goals related to behavioral health.
• Provide direct interventions using the I-FAST treatment model and principles.
• Conduct a thorough assessment of the client and family to inform problem behaviors and interactions.
• Conduct treatment planning and collaborate with schools, courts, care management entities, etc.
• Provide care in home and community-based settings.
• Engage primary caregiver and supports in change-oriented treatment.
• Weekly case planning and evaluation of case progress with supervisor support.
• Attend regular training, supervision, and consultation activities to gain clinical skills.
• Provide backup for team members to ensure continuity of care and 24/7 coverage.
• Adhere to EHR documentation policy and ISBH standards.
• Travel within designated service areas.
• Other duties as assigned.
