Job summary

BestCare Treatment Services seeks an Intellectual/Developmental Disabilities Services Coordinator to coordinate services for individuals with IDD. You will assess needs, plan care, make referrals, and authorize services, collaborating with guardians, families, and community partners.

The role requires driving within Oregon, case management, and adherence to state regulations. Prior experience with IDD and a valid driver license are essential.

Qualifications

• Bachelor’s degree in behavioral or social sciences or closely related field or equivalent.
• Experience with individuals who have intellectual or developmental disabilities.
• Valid Oregon driver license and ability to travel as needed.

Responsibilities

• Conducts assessments and level of care, planning documentation and case management contacts.
• Completes clinical documentation for services and ensures compliance with state rules.
• Coordinates services with social service agencies, medical providers and other bodies.
• Provides case management in accordance with Oregon Administrative Rules in various settings.
• Attends required trainings within 90 days and maintains ongoing training hours.
• Facilitates annual needs assessments and LOC approvals for Medicaid funded services.
• Ensures client choice of services and maintains annual rights notification.
• Assists clients in obtaining home and community-based services and resources.

Education

Bachelor’s degree in behavioral science, social science, or closely related field

Bachelor’s degree in any field plus one year of human services related field

Associate’s degree in behavioral science, social science, or closely related field plus 2 years of human services-related experience

At least three (3) years of human services related experience

Experience working with individuals with intellectual or developmental disabilities

Must maintain a valid Oregon Driver License and be insurable; travel as needed

Bilingual in English/Spanish a plus
