Job summary

Support, Inc. is seeking a Clinician in Colorado to provide counseling and consultation to adults with intellectual and developmental disabilities within Medicaid Waiver services.

The role partners with an interdisciplinary team to tailor interventions that fit each client's needs and community-based goals. You will work across homes and community settings, contribute to treatment planning, train families, and help advance regional initiatives while meeting monthly productivity targets.

Qualifications

• Must hold a relevant Master's degree and be eligible for CO license.
• Experience with clinical populations, preferably in I/DD.

Responsibilities

• Providing consultation and counseling to people with I/DD under Medicaid Waiver.
• Conducting assessments and formulating multidisciplinary treatment plans for homes and community settings
• Consulting with and training family members and host-home providers on interventions
• Collaborating with IDT to provide wholistic support to people receiving services
• Meeting monthly billed unit productivity goals
• Participating in regional leadership team meetings and planning regional events
