Job summary

UnitedHealth Group is seeking a Telephonic Case Manager RN in Georgia. This hybrid role begins with onsite training and transitions to work-from-home with periodic onsite meetings in Sandy Springs.

You will manage education, advocacy, and care coordination for a diverse member population, ensuring access to resources and adherence to treatment plans. The ideal candidate holds an active Georgia RN license, at least 3 years of relevant experience, and is comfortable traveling to the Sandy Springs

Qualifications

• Active, unrestricted RN license in GA.
• 3+ years RN experience in hospital/acute care or telephonic case management.
• Willingness to travel to the Sandy Springs office as needed.
• Strong computer/data entry skills and Windows proficiency.
• Ability to participate in on-site training and hybrid work schedule.

Responsibilities

• Make outbound and inbound calls to assess members' health status.
• Identify gaps or barriers in treatment plans and coordinate care.
• Educate members on disease processes and self-management.
• Collaborate with Medical Directors on challenging cases.
• Document findings and track member progress.
• Conduct post-discharge calls to ensure services are in place.
