Job summary

Mosaic in Kansas is seeking a Mosaic at Home Coordinator to empower individuals with intellectual and developmental disabilities to live fulfilling lives. Based in Topeka & Manhattan, KS, you will build connections with individuals, families, home providers, and stakeholders to deliver personalized support.

You will help people articulate goals, monitor matches, and develop Individual Support Plans while ensuring policy and regulatory compliance.

Qualifications

Bachelor's degree preferred.
Minimum of two years of related work experience.
Experience working with people with intellectual disabilities preferred.

Responsibilities

Build meaningful connections with individuals, families, guardians, home providers, and other stakeholders.
Help individuals express goals and needs, ensuring their voices are heard.
Monitor relationships between individuals and home providers, ensuring a positive match.
Develop Personalized Support Plans reflecting each person's wishes and needs while adhering to policy and regulatory requirements.
Oversee contracted services and complete visits with individuals and providers in home or service environments.
Provide on-call support to Mosaic at Home Providers and ensure consistent support and problem resolution.
