Job summary

Church World Service, Inc. is seeking a Case Manager to provide direct services and holistic support to clients facing complex challenges.

You will conduct intake assessments, develop service plans, and coordinate referrals while ensuring trauma-informed, culturally responsive care. Responsibilities include maintaining confidential records, advocating with external agencies, linking clients to housing, healthcare, employment, and education, and collaborating with program teams.

Qualifications

• Bachelor's degree in social work or related field, or equivalent experience.
• Minimum 3 years of case management or direct service experience.
• Trauma-informed, culturally responsive care expertise.
• Strong written and verbal communication.
• Knowledge of local community resources and social service systems.
• Proficiency with Microsoft 365 and case management software.

Responsibilities

• Conduct intake assessments to determine client needs, risks, and eligibility for services.
• Develop individualized service plans with measurable goals and timelines.
• Provide ongoing case management, monitoring progress, and adjusting plans as needed.
• Facilitate access to housing, healthcare, employment, education, legal support, and financial assistance.
• Advocate for clients with external agencies to secure necessary resources.
• Maintain accurate, confidential case files and track outcomes for reports.
• Coordinate services with PIR, Matching Grant, and other teams to enroll clients.
