Job summary

Advocate Health seeks a Speech-Language Pathologist for its Home Care Speech Therapy program in Wisconsin. The role involves direct patient care both in person and via telehealth, with a Monday–Friday schedule from 8:00 am to 4:30 pm and no weekends or holidays.

You will evaluate and treat a range of disorders, maintain records, and collaborate with physicians. Requires a Master’s in Speech Pathology, Wisconsin SLP license, BLS, and a valid driver’s license.

Qualifications

• Master's Degree in Speech Pathology.
• Typically requires 3 years of experience in speech therapy within acute hospital or home health care setting.
• Wisconsin SLP licensure and BLS certification required; valid driver’s license.

Responsibilities

• Conducts speech and language evaluations and develops plan of care.
• Evaluates outcomes within a defined timeframe.
• Treats speech, language, voice and swallowing disorders per physician prescription.
• Communicates treatment plans and updates to physicians and management.
• Maintains accurate clinical and patient records per state/federal guidelines.
• Manages caseload including scheduling and coordination with site leadership.
• Provides patient advocacy and ongoing assessment of treatment responses.
• Discharges patients with written home programs and caregiver training.
