Job summary

Continuum Therapy Partners, LLC is seeking a Home Health Speech-Language Pathologist to join our Milwaukee, Wisconsin area team on a PRN basis. A Master’s Degree in Speech-Language Pathology, current Wisconsin license (CCC-SLP preferred), and strong clinical and communication skills are required; 1 year of

Qualifications

• Master’s Degree in Speech-Language Pathology required.
• Current, unencumbered state SLP License; CCC-SLP required.
• 1 year of previous experience as an SLP preferred.

Responsibilities

• Evaluates and develops individualized treatment plans for speech, language, and swallowing disorders.
• Documents patient progress and adjusts treatment plans based on clinical outcomes.
• Educates patients and families on communication strategies and swallowing safety.

Speech-Language Pathologist Requirements & Responsibilities

• ?Master’s Degree in Speech-Language Pathology required
• ?Current, unencumbered state SLP License; ASHA Certificate of Clinical Competence (CCC-SLP) required
• ?1 year of previous experience as a Speech-Language Pathologist preferred
