Qualifications

• Bachelor's Degree from an accredited school of social welfare or related human services field preferred.
• Demonstrated interest in working with persons with developmental disabilities.
• Access to reliable transportation.

Responsibilities

• Assess the needs of developmentally disabled individuals and develop Individual Program Plans.
• Coordinate and monitor services and manage a wide range of active cases.
• Meet with individuals at residence or in the community to implement and revise plans.
• Prepare required reports and documentation in a timely and accurate manner.
