Job summary

North Bay Regional Center is seeking a Social Services Case Manager to coordinate services for individuals with developmental disabilities in Napa, Sonoma and Solano counties. The role centers on guiding clients toward their IPP goals and leveraging community resources to promote independence.

You will work within our Service Coordinators team, focusing on person-centered planning and coordinated care across multiple agencies, implementing plans, and reassessing client needs.

Qualifications

• Bachelor’s degree in a field related to developmental disabilities (Social Work, Psychology, Counseling, Special Education, Nursing)
• Master’s degree preferred
• Minimum 3 years’ experience working with individuals with developmental disability
• Bilingual language ability (Spanish to English) preferred

Responsibilities

• Use person-centered planning to help clients access public and private resources before using regional center funds
• Monitor vendor services and verify services align with purchase of service authorizations
• Evaluate the effectiveness of clients’ Individual Program Plans and connect them with appropriate community resources
• Conduct annual reassessments of clients’ developmental status with the interdisciplinary team
• Perform client assessments on an occasional basis
