• Investigates and/or reports abuse, neglect, or exploitation of adults and/or children with impairments to determine need for protective services.
• Implements a service plan that addresses needs or concerns of the client(s) and coordinates services to reduce risk and increase autonomy.
• Identifies risk factors while assessing clients and monitors progress of service plans.
• Documents services provided, client contacts, observations, and progress in client files and databases.
• Participates on a multi-disciplinary team to strategize solutions and uses required equipment to assess or implement client services.
• Educates government departments, local agencies, law enforcement, the public, and external entities about agency programs.
