Qualifications

• Education required: bachelor's degree plus one year of professional experience in health care for children, adolescent, or geriatric populations, or related social work/counseling fields.
• Graduate coursework may substitute for experience on a year-for-year basis, up to one year.

Responsibilities

• Investigates and reports abuse, neglect, or exploitation of adults/children with impairment to determine protective needs.
• Implements a service plan and coordinates services to reduce risk and increase autonomy.
• Identifies risk factors as part of client assessment and monitors progress of service plans.
• Documents services, contacts, and progress in client records and databases.
• Participates in multidisciplinary team to strategize client solutions and uses necessary equipment to assess or implement services.
• Educates government departments and external entities about agency programs.
