Job summary

Alleghenies United Cerebral Palsy is hiring a Service Coordinator to develop and manage personalized service plans for individuals, coordinating with providers and community resources to ensure support is available and appropriate.

You will work in a client-centered role requiring strong communication, coordination, and compliance skills, with a hybrid/remote PA position and travel within the state as needed.

Qualifications

• Bachelor's degree required in Social Work, Psychology, Education, Human Services, or related field.
• At least 2–3 years of professional case management experience for people with physical disabilities.
• At least 2–3 years of experience working with individuals with Autism.
• Knowledge of Medicaid Waiver programs, LTSS, and HCBS preferred.

Responsibilities

• Develop, update, and manage individualized service plans (ISP) based on client goals.
• Coordinate with paid providers and community resources to meet client needs.
• Maintain open communication with support teams for timely updates and concerns.
• Ensure compliance with AUCP standards, documentation, timelines, and billing requirements.
• Travel within the assigned region to meet with participants as needed.
