Job summary

Alleghenies United Cerebral Palsy (AUCP) is seeking a Service Coordinator to work with individuals and their support teams to develop, update, and manage personalized service plans. The role emphasizes client-centered coordination, collaboration with providers, and ensuring services meet approved ISP goals.

The position is Hybrid/Remote with community visits in PA. A Bachelor's degree in a related field is required, and Medicaid/LTSS knowledge is preferred.

Qualifications

• Bachelor's degree or higher in a related field is required.
• Knowledge of Medicaid Waiver programs, LTSS, HCBS is preferred.
• Experience working with individuals with disabilities or special needs is desired.

Responsibilities

• Develop individualized service plans (ISP) with clients and teams.
• Regularly update service plans based on life changes and needs.
• Coordinate services with paid providers and community resources to ensure plans are implemented correctly.
