Job Description

We're hiring for Primary Care Chronic Care Managers.

Education/Experience Required
  • Active RN or LPN license (compact)
  • Two (2) years of prior long-term care service coordination, care delivery monitoring, and care management experience. This experience may be concurrent with the two years of experience working with the geriatric population described above.
  • Must reside in North Carolina or South Carolina
  • Must have the ability to travel regularly as needed to perform job duties
Qualifications
  • Experience working with members/recipients with co-occurring physical health and/or behavioral health needs is preferred.
Work Environment/Travel

Hours of operation are Monday-Friday 8am-5pm. This is a hybrid position requiring a private, quiet, HIPAA-compliant workspace, and onsite travel to regional facilities.

LifeSource seeks a customer service-focused Chronic Care Manager to join our Primary Care Team, vital for providing quality healthcare and improving patients' quality of life. This role involves working both onsite and remotely with patients and providers, requiring an upbeat personality, focus, and excellent care delivery skills.

Responsibilities
  • Discuss benefits of chronic care management with patients and families, and obtain enrollment.
  • Complete Health Risk Assessments and determine RSCM scores as needed.
  • Coordinate and provide client-centered chronic care management discussions.
  • Draft care plans, review case progress, and handle case assignments.
  • Facilitate referrals, imaging, labs, medication refills, and adherence.
  • Collaborate with the medical team throughout case management.
  • Accurately document case information and produce reports.
  • Help clients achieve wellness and autonomy.
  • Coordinate with healthcare providers and long-term care communities.
  • Engage with patients to monitor progress and ensure satisfaction.
  • Promote quality, cost-effective interventions and outcomes.
  • Address motivational and psychosocial issues.
  • Adhere to professional standards and regulations.
