Job Description

We're hiring for Primary Care Chronic Care Managers.

Qualifications
  • Active RN or LPN license (compact)
  • Two (2) years of prior long-term care service coordination, care delivery monitoring, and care management experience. This experience may be concurrent with the two years of experience working with the geriatric population described above.
  • Must reside in North Carolina or South Carolina
  • Must have the ability to travel regularly as needed to perform job duties
Work Environment & Travel

Hours are Monday-Friday, 8am-5pm. This is a hybrid position requiring a private, quiet, HIPAA-compliant workspace and onsite travel to regional facilities.

Responsibilities
  • Engage with patients and families to discuss benefits of chronic care management and facilitate enrollment.
  • Complete Health Risk Assessments and determine RSCM scores as necessary.
  • Coordinate and provide timely, effective, and patient-centered chronic care management discussions.
  • Handle case assignments, draft care plans, and review case progress with healthcare providers.
  • Manage referrals, imaging, labs, medication refills, and adherence.
  • Collaborate with the medical team throughout case management.
  • Document case information accurately and produce reports.
  • Assist clients in achieving wellness and autonomy.
  • Coordinate with healthcare providers, long-term care facilities, and clinical teams.
  • Maintain communication with patients to monitor progress and satisfaction.
  • Promote quality, cost-effective care interventions.
  • Address motivational and psychosocial issues.
  • Follow professional standards and regulations.

  • Knowledge of HIPAA and medical terminology

Must frequently communicate with facilities, patients, and providers regarding patient needs, complaints, and inquiries.
