Job Description

We're hiring for Primary Care Chronic Care Managers.

EDUCATION/EXPERIENCE REQUIRED:
  • Active RN or LPN license (compact)
  • Two (2) years of prior long-term care service coordination, care delivery monitoring, and care management experience. This experience may be concurrent with the two years of experience working with the geriatric population described above.
  • Must reside in North Carolina or South Carolina.
  • Must have the ability to travel regularly as needed to perform job duties.
Additional Experience:

Experience working with members/recipients with co-occurring physical health and/or behavioral health needs.

WORK ENVIRONMENT/TRAVEL

Hours of operation are Monday-Friday 8am-5pm. Standard office equipment is provided and required to perform job duties. This is a hybrid position, requiring the candidate to have a private, quiet, HIPAA-compliant workspace, and to travel onsite to regional facilities as needed.

The successful candidate will work with onsite and remote providers to serve patients needing our services. DUTIES/RESPONSIBILITIES

The following are expected duties but are not all-inclusive; other duties may be assigned:

  • Engage with new and established patients to discuss benefits of chronic care management and facilitate enrollment.
  • Complete Health Risk Assessments and determine RSCM scores as necessary.
  • Coordinate and conduct safe, timely, effective, and equitable chronic care management discussions centered on the patient.
  • Handle case assignments, develop care plans, and review case progress with healthcare providers.
  • Facilitate care coordination, including referrals, imaging, labs, medication refills, and adherence.
  • Collaborate effectively with the medical team throughout case management.
  • Accurately document case information and produce statistical reports.
  • Assist clients in achieving wellness and autonomy.
  • Coordinate with healthcare providers, long-term care communities, and clinical organizers as needed.
  • Interact with patients to monitor progress and ensure satisfaction.
  • Promote quality, cost-effective interventions, and positive outcomes.
  • Assess and address motivational and psychosocial issues.
  • Adhere to professional standards, protocols, rules, and regulations.
Skills and Qualifications
  • Customer service and phone experience.

  • Prior healthcare scheduling or medical experience.
  • Knowledge of HIPAA laws and medical terminology.
  • Strong computer skills (Microsoft Word, Excel, Outlook, and general Microsoft and Apple products).
  • Drive to help patients and staff with tact and diplomacy.
  • Clear and concise communication skills, both verbal and written.
  • Strong customer service attitude.
  • Reliable vehicle for frequent communication and travel.

Must be able to communicate effectively with facilities, patients, and others regarding requirements, complaints, and inquiries.
