Job summary

Ponos Care's RN Case Manager role in the Northern VA area blends telephonic case management with infusion therapy in patients' homes. You will assess, plan, coordinate high-quality, evidence-based care across states, collaborate with providers and families, and ensure regulatory compliance while reducing hospital readmissions. Travel to patient homes is required as needed in a hybrid telephonic model to support Medicaid populations.

Qualifications

• BSN required and RN license active in all states of practice.
• Minimum 2+ years of clinical nursing experience in care management, infusion therapy, or related fields.
• CCM certification preferred; ACLS/ONCC/CRNI desirable.
• Experience with Medicaid populations and population health.
• Strong communication, critical-thinking and remote/hybrid work skills.

Responsibilities

• Conduct comprehensive assessments via telephonic outreach or in-home visits.
• Develop and implement personalized care plans with interdisciplinary teams.
• Provide infusion therapy and acute care in patients' homes when needed.
• Coordinate services, monitor progress, and address barriers to care.
• Maintain thorough documentation meeting CMS/NCQA/HEDIS standards and HIPAA.
