Job summary

Utah Case Management in Utah is seeking a Nurse Case Manager to coordinate Medicaid NCW program services. This role focuses on reviewing medications, assessing health needs, and coordinating supports to protect client health and safety.

The position is hybrid with remote documentation and in-person visits across Salt Lake County. You will work with Social Work Case Managers, facility staff, and providers; complete nursing assessments, maintain records, and ensure compliance.

Qualifications

• Utah resident living in Utah county.
• Current Registered Nurse (RN) license in Utah, in good standing.
• Ability to travel throughout assigned county for client visits.
• Strong communication, organizational, and documentation skills.
• Ability to work independently and collaborate with a team.

Responsibilities

• Complete medication management reviews and document findings.
• Assess health conditions and identify supports to maintain health and safety.
• Coordinate services and follow up on changes in health status.
• Collaborate with Social Work Case Managers, facility staff, and service providers.
• Maintain accurate nursing assessments and ensure timely documentation.
• Protect client confidentiality and comply with program policies.
