Job description

The Dual Nurse Case Manager combines nursing and social work responsibilities to coordinate medical services and care for clients participating in Utah's Medicaid New Choices Waiver program. This role works collaboratively with clients, families, service providers, facilities, and other members of the care team to help coordinate and manage the services each client needs.

This is a hybrid position with remote documentation and in-person client visits. The position offers a flexible work schedule, allowing the Case Manager to govern their own hours and schedule while meeting client and program needs.

Nursing Responsibilities

Complete nursing assessments, including MDSHC, LOC, and Medication Management Reviews, within the required time frames for each client.

Collaborate with the social worker on healthcare decisions and healthcare needs for clients.

Complete health status screening reports when clients return from an inpatient setting or experience a decline in health status.

For clients requiring memory care, complete the memory care checklist and required documentation for approval to reside in memory care.

Attend required nursing training meetings and other required trainings.

Social Work Responsibilities

Maintain weekly contact with clients, service providers, family members, and other individuals involved in the client's care.

Complete required monthly face-to-face contact with clients.

Assess, coordinate, and manage needed services for clients.

Complete all initial care plans in pending or approved areas within the State Online Care Plan system by the 5th of the month following enrollment.

Review and re-submit denied care plans within 48 hours of denial.

Complete all required documentation, including audits and wrap-up lists.

Conduct new referrals, annual reviews, settings reviews, and other required case management activities.

Dual Role Responsibilities

Accept constructive criticism and direction and apply feedback to the role.

Work independently while also functioning effectively as part of a team.

Use a personal computer and phone for work purposes.

Complete log notes by the 15th of the month for the first half of the month and by the last day of the month for the second half of the month.

Attend monthly team meetings and case reviews with supervisors as needed.

Respond to facilities regarding new referrals within 24-48 hours of receiving the referral to confirm awareness of the client and communicate when the Case Manager will meet with the client.

Qualifications

Utah resident residing in or near Vernal, Utah.

Current Registered Nurse license in the State of Utah, maintained in good standing.

Be available during some business hours.

Ability to create and maintain positive rapport with clients.

Strong attention to detail.

Self-motivated and able to work independently.

Strong communication and organizational skills.

Reliable access to a personal computer/device for work and documentation.
