Job summary

CVS Health, as part of Aetna One Advocacy (A1A), is seeking a Registered Nurse Case Manager, requiring in-office training and later work-from-home with occasional in-office days. Must live within a 45-minute radius of New Albany, OH, with 8am-4:30pm EST hours and a November 9, 2026 cohort start.

Requirements include an RN license, 3+ years of clinical practice, and associates degree in nursing (BSN preferred). CCm, Oncology, bilingual skills, and Diabetes care experience are valued.

Qualifications

• RN with unrestricted license in home state; multi-state/compact preferred.
• Willing to obtain licenses in all 50 states.
• 3+ years of clinical practice as an RN.
• Must live within 45 minutes of New Albany, OH.
• CCM certification preferred.
• Oncology experience preferred.
• Bilingual in English and another language preferred.
• Diabetes Care and Education Specialist preferred.
• Associates Degree in Nursing required; BSN preferred.

Responsibilities

• Develops proactive actions to improve short and long-term outcomes and overall wellness.
• Evaluates member needs and eligibility and facilitates transitions to programs.
• Applies clinical judgment to reduce risk factors and barriers in care planning.
• Assesses information from multiple sources to address co-morbid conditions.
• Reviews prior claims to assess impact on case management.
• Assesses work capacity and related restrictions.
• Refer to clinical resources as needed to determine functionality.
• Consults with supervisor to overcome barriers and present cases for team focus.
• Adheres to regulatory and company policies in case management.
• Uses interviewing skills to engage members and assess health needs.
