CVS Health’s Aetna One Advocacy team in New Albany, Ohio seeks a Registered Nurse Case Manager. The role starts with in-office training for ~4 months, then transitions to a work-from-home model with occasional office days.

Qualifications

Registered Nurse with an unrestricted license and multi-state/compact privileges preferred.
Willing to obtain licenses in all 50 states.
3+ years of clinical RN experience.
Must live within a 45-minute commute of New Albany, Ohio.

Responsibilities

Assess, plan, implement and coordinate case management activities for members.
Evaluate medical needs to facilitate overall wellness and program eligibility.
Use clinical judgment to reduce risk factors and barriers in care planning.
Coordinate with supervisors and teams during case conferences for comprehensive care.
Ensure compliance with regulatory and company policies in case management.
