Job summary

Heritage Health Network (HHN) seeks a Registered Nurse Care Manager (RNCM) to provide clinical oversight for the ECM program, guiding non-clinical Lead Care Managers. The role blends remote clinical work with field-based activities and in-person member engagement as needed.

The RNCM will review charts, assess plans, identify gaps, and ensure high-quality care coordination for ECM members. Field visits and collaboration with providers are essential.

Qualifications

• Active RN license in California.
• Minimum 2 years of nursing experience.
• At least 1 year in care management, case management, or leadership.
• Experience with vulnerable populations and complex needs.
• Strong clinical assessment and care planning skills.
• Proficiency with EMR/EHR systems.
• Excellent communication and collaboration skills.
• Ability to perform field-based work and in-person engagement.

Responsibilities

• Provide clinical oversight to ECM program and Lead Care Managers.
• Review member charts, assess care plans, and identify gaps in care.
• Guide complex cases and ensure high-quality coordination.
• Conduct field-based visits and member engagement activities.
• Coordinate with providers, hospitals, and community organizations.
• Support care transitions and discharge planning.
• Participate in weekly case reviews.
