Bluestone Physician Services in Minnesota is seeking a Care Coordinator to help senior clients navigate their healthcare journey. You will manage DHS and CMS requirements for MSHO/MSC+ members and perform home visits with remote work balance.

This hybrid role offers 50–70% local visits and 30–50% remote work, with a schedule of 8:00 AM–5:00 PM (Mon–Thu) and 8:00 AM–3:00 PM (Fri), no evenings or weekends. Salary range $65,000–$75,000 per year, depending on experience.

Qualifications

• Ability to work independently in a home-based office setting.
• Excellent communication and interpersonal skills.
• Experience in geriatric care coordination or related field preferred.

Responsabilités

• Coordinate face-to-face visits with clients in their homes or communities.
• Manage DHS and CMS requirements for MSHO/MSC+ members.
• Conduct annual assessments and develop individualized care plans.
• Collaborate with care teams to ensure quality and safe discharge plans.
• Document care activities and maintain compliance with regulations.
