This Licensed Clinical Social Worker role supports the behavioral health care team to meet members' mental health needs across designated offices, with a hybrid remote/Brevard County setup.

The position emphasizes collaboration with therapists, health coaches, and care partners to deliver equitable, value-based care. Prior social work experience in behavioral health is preferred.

Qualifications

• Master's degree in social work (MSW) is required.
• State Licensure as a Clinical Social Worker is required.
• Specialized certification as Licensed Clinical Social Worker (LCSW) is required.

Responsibilities

• Support clinical care team with SBIRT implementation.
• Engage in care coordination to access therapy and psychiatry partnerships.
• Collaborate with a multidisciplinary care team to serve members.
• Provide Social Determinants of Health screenings, resources, and referrals.
• Engage in community outreach to build relationships with local organizations.
• Coordinate with local substance use treatment programming for member assessments and referrals.
• Provide education for members and employees on current standards of care in mental health.
