Job Details

We are hiring an RN with a master’s degree and at least 3 to 5 years of Hospice / Palliative Care experience, with Supervisory or Leadership experience, to work as our Hospice RN Team Manager.

The Team Manager supports the Hospice Clinical Director by providing leadership guidance to the professional staff on a day-to-day basis, ensuring the provision of hospice services aligns with the philosophy, goals, and policies of the organization.

Scope of Responsibility:

• Monitor and evaluate the quality and completion of nursing documentation per policy and regulations.
• Facilitate weekly IDT team meetings to review all patients per Hospice Conditions of Participation (COPs).
• Ensure departmental operations stay within financial guidelines using the net patient revenue (NPR) of the MVI model, including staff KPIs.
• Implement and evaluate the Hospice Performance Improvement Plan related to NHPCO Family Satisfaction Survey results.
• Participate in referral and community education to enhance hospice awareness.
• Manage performance of direct and indirect reports, providing coaching, training, development, and corrective actions as needed.
• Ensure appropriate staffing and resources.
• Monitor productivity and participate in annual budget preparation.
• Assist in hiring qualified staff in collaboration with HR and management.
