PeopleOne Health, Inc. is seeking a Registered Dietitian to engage with members in-person, virtually, or by telephone to provide medical nutrition therapy. The role supports a hybrid/remote model serving Polk County and surrounding areas.

Responsibilities include nutrition assessments, diet planning, and counseling to improve health outcomes. Requires CDR registration, a nutrition-focused degree, and state licensure, with 1–2 years clinical experience preferred. Travel to sites may be required.

Formación

• Requires a Bachelor's degree or higher in nutrition (Master's preferred).
• CDR registration and state licensure required.
• 1–2 years clinical setting experience.
• Experience with medical nutrition therapy and chronic disease management.

Responsabilidades

• Conduct nutrition assessments to identify needs and plan care.
• Provide evidence-based nutrition therapy considering medical history and social factors.
• Counsel individuals and groups on healthy eating to improve quality of life.
• Collaborate with physicians and interdisciplinary team to coordinate care.
• Document assessments and plans in EMR according to protocol.
• Serve as nutrition liaison to assist on company initiatives.
• Demonstrate clear written communication adaptable to various health literacy levels.
