• Improve communication between community behavioral health providers and pediatric PCPs.
• Ensure understanding of recommendations by following up with providers, clients, or families.
• Document and provide behavioral health resources as advised by treating psychiatrist.
• Organize care activities and share information among participants.
• Integrate medical and social services for comprehensive care.
• Provide social work services to help patients benefit from treatment.
