Johns Hopkins Bayview Medical Center is seeking a clinically engaged Rehabilitation Liaison to support admission decisions and coordinate care across disciplines. This hybrid, full-time role connects patients, families, referring providers, and the rehabilitation team to ensure timely access to services.

The position emphasizes collaboration, patient advocacy, and education, with opportunities to work across a nationally recognized academic medical center and Level II trauma center.

Qualifikationen

• Active Maryland PT, OT, SLP, or RN license.
• Bachelor's degree in a related field.
• Current Healthcare Provider CPR certification.
• Experience in rehabilitation, acute care, care coordination, admissions, case management, or a related healthcare setting.

Aufgaben

• Review patient referrals and gather clinical information to support admission decisions.
• Collaborate with physicians, case managers, discharge planners, and clinical teams to facilitate rehabilitation admissions.
• Educate patients and families regarding rehabilitation services and expectations.
• Coordinate communication between referral sources and internal clinical teams.
• Support seamless transitions of care across the continuum.
• Maintain accurate referral, evaluation, and admission documentation.
• Build strong relationships with referral sources and community partners.
• Serve as a clinical resource throughout the admission process.
