Hutchinson Regional Medical Center is seeking a Clinical Liaison for Rehabilitation to build relationships with healthcare providers, patients, and families to promote referrals and ensure access to inpatient rehabilitation care.

You will collaborate with care teams, verify insurance, educate patients and families, and document care steps to support positive patient outcomes throughout the rehabilitation journey.

Formación

• Education commensurate with license requirements in Kansas.
• Experience in care management or clinical settings.
• Current BLS/CPR within 30 days of hire.

Responsabilidades

• Build effective relationships with patients, families and providers; promote referrals.
• Ensure access to high-quality inpatient rehabilitation services.
• Coordinate pre-screening and verify insurance information for referrals.
• Maintain compliant documentation per policy.
• Advocate for patient needs and support throughout rehab process.
• Collaborate with internal/external stakeholders to improve outcomes.
