Job summary

P3 Health Partners is seeking a Regional Quality Coordinator in Roseburg, OR. The role focuses on HEDIS and quality gap closures, coordinating across clinics to improve care and outcomes.

The position requires data-driven problem solving, telephonic outreach, and stakeholder engagement with internal teams and providers. You will support CMS/HEDIS initiatives, manage projects like Quality Fairs, and interact with patients to drive education, adherence, and preventive screenings.

Qualifications

• High school diploma or GED required; associate degree preferred.
• 2+ years in healthcare or health plan settings preferred.
• Proficient in MS Word/Excel/PowerPoint/Outlook; EHR experience preferred.

Responsibilities

• Provide telephonic outreach to patients, providers, and pharmacies to discuss opportunities for improving care and adherence.
• Support Quality Management activities and assist in data collection for quality measures.
• Coordinate across clinics and departments to close quality gaps (HEDIS, CMS Star ratings).
• Lead project work for quality events and initiatives to improve care delivery.
• Prepare quality information for affiliate groups and maintain documentation for audits.
