Job summary

P3 Health Partners is seeking a Healthcare HEDIS/Quality Operations Coordinator in Medford, OR. The role focuses on supporting HEDIS and pharmacy gap closures with daily problem solving and cross-department coordination, including patient outreach for education and engagement on preventive screenings.

The Regional Quality Coordinator will assist the market VP and National Quality Manager, travel within the region, and interact with providers and patients to improve quality measures and care

Qualifications

High school diploma required; associate degree or related field preferred.
2+ years in healthcare-related roles; managed care or health plan experience preferred.
Proficient with Word, Excel, PowerPoint, Outlook; EHR experience required.
Medical terminology knowledge; CPT II codes preferred.

Responsibilities

Support CMS, HEDIS, NCQA based quality initiatives and reporting.
Provide telephonic outreach to patients, providers, and pharmacies for education and engagement on preventive screenings.
Track quality metrics and assist in quality program management across affiliates.
Plan and execute Quality events (Fairs, Diabetic Days, P3 events) to close gaps.
