Job summary

Concept Rehab, Inc in Willoughby, OH is seeking a full-time Physical Therapist to join our team. The PT will evaluate patients, develop individualized plans, and provide skilled interventions across inpatient and outpatient settings to improve mobility and independence.

Collaborate with physicians and nurses, document evaluations and progress, and support patient-focused care. Ohio license and graduation from an accredited PT program required; inpatient/outpatient experience preferred.

Qualifications

• Current Ohio Physical Therapist license.
• Graduate of an accredited PT program.
• Inpatient/outpatient experience preferred.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide skilled physical therapy interventions.
• Treat patients across inpatient and outpatient settings.
• Improve strength, balance, mobility, and functional independence.
• Collaborate with physicians, nursing, therapy, and other healthcare professionals.
• Accurately document evaluations, treatments, and progress.
