Job summary

Geode Health is seeking a Mental Health Provider (Psych PA or PMHNP) to join our hybrid model in the greater West Town market. You will conduct initial evaluations, develop individualized treatment plans, and provide medication management with supportive psychotherapy as needed.

You will work closely within an integrated care team to coordinate care. The role emphasizes balance—no nights, weekends, or on-call, with a flexible schedule that includes onsite and telehealth days.

Qualifications

• Active Psychiatric PA or PMHNP license in IL.
• Geode requires one year of professional work experience for Psych PA’s.
• Active DEA license.

Responsibilities

• Provide psychiatric healthcare services including initial evaluations and treatment planning.
• Medication management and supportive psychotherapy as clinically appropriate.
• Collaborate with an integrated care team to coordinate care.
