Job summary

Blackbird Health is seeking an Intake Therapist to deliver high-quality intake evaluations for youth (ages 2–26) in a hybrid setting, using evidence-based practices and tailored assessments. You will establish early connections with patients and families, educate them about treatment options, and ensure a smooth transition into ongoing care.

You will collaborate with an interdisciplinary team to create individualized care plans, drive appropriate provider matching, and maintain detailed intake

Qualifications

• Active independent license as an LPC, LMFT, or LCSW in Pennsylvania.
• Minimum 1 year of experience with child and adolescent patients.
• Experience in telehealth or virtual settings.

Responsibilities

• Deliver high-quality intake evaluations for youth via in-person and telehealth.
• Build meaningful first connections with patients and families.
• Drive strong clinical matches with individualized care plans.
• Collaborate with clinicians and families for integrated care.
