Job summary

Blackbird Health is seeking an Intake Therapist to conduct high-quality intake evaluations for youth in a hybrid in-person/telehealth setting. You will engage families from the start, educate them about therapeutic options, and ensure smooth transitions into ongoing care with a coordinated, interdisciplinary team.

The role supports afterschool, evening, and weekend intake sessions, requiring flexibility and strong clinical assessment skills.

Qualifications

• Active independent license as an LPC, LMFT, or LCSW in Pennsylvania.
• Minimum 1 year of experience working with child and adolescent patients.
• Experience delivering intake evaluations and telehealth/virtual care.

Responsibilities

• Lead individual in-person and telehealth intake sessions for youth ages 2–26 using evidence-based assessment practices.
• Build meaningful connections with patients and families to understand needs and educate them about therapeutic approaches.
• Develop individualized care plans ensuring appropriate ongoing treatment and provider matches.
• Collaborate with clinicians, medical providers, and families to create coordinated treatment plans addressing the whole child.
• Participate in weekly paid clinical consultations to enhance clinical skills and maintain thorough intake documentation.

Education

Active independent license as LPC/LMS/LCSW in Pennsylvania
