Job summary

Somatus seeks a Patient Health Advocate to support high-need CKD and ESKD patients, working with physician practices and care teams to coordinate resources and care plans. The role blends in-office physician settings with home visits in a hybrid telehealth model.

Responsibilities include patient outreach, enrollment support, and collaboration with RNCM and care teams to improve outcomes and reduce readmissions. Bilingual skills are a plus in serving diverse populations.

Qualifications

• High school diploma or equivalent required.
• 1+ years of case management or care management experience.
• 2+ years of healthcare-related experience.
• Experience with Medicare, Medicaid, and/or Special Needs populations.
• Experience working in a physician office.

Responsibilities

• Support care management coordination committee meetings.
• Serve as primary contact for provider practices regarding patient needs.
• Assist patients during transitions of care and scheduling follow-ups.
• Collaborate with physicians and care team to ensure care plans are implemented.
• Engage with patients to facilitate enrollment and ongoing self-care.
• Maintain HIPAA-compliant documentation and use care coordination platforms.
