Job summary

Somatus is seeking a Patient Health Advocate to support high needs CKD/ESKD populations. You will partner with physician practices, attend care team meetings, and build strong relationships with providers to coordinate patient care and enrollment in Somatus programs.

Responsibilities include outreach, care coordination, and education, with travel to provider offices as needed and a focus on improving care transitions and patient outcomes.

Qualifications

• 1+ years of case management or care management experience.
• 2+ years healthcare related experience.
• Experience working with Medicare/Medicaid and/or Special Needs populations.
• Experience working in a physician office.

Responsibilities

• Serve as primary contact for provider practice regarding patient needs and care coordination.
• Collaborate with physicians and practice staff to improve patient care and support clinical and operational goals.
• Assist in preparing and managing regular interdisciplinary care team meetings.
• Document activities using the care coordination platform in collaboration with physician practices.
• Support patients during transitions of care to facilitate effective handoffs and reduce readmissions.
• Coordinate scheduling for follow-up and initial assessments with care team members.
• Educate patients about Somatus programs and reinforce care plans and workflows.
• Engage with patients to overcome language or cultural barriers and support self-care.
