Job summary

Somatus in the United States is seeking a Patient Health Advocate (PHA) to support high-need CKD and ESKD patients with care coordination, resource access, and adherence to treatment plans. The PHA will work closely with physician practices, spend time in offices and in patients' homes, and serve as the primary Somatus representative to build trust and foster collaborative care.

Qualifications

• High school diploma or equivalent required.
• 1+ years of experience in case management or care management.
• 2+ years healthcare related experience.
• Experience working with Medicare, Medicaid, and/or Special Needs populations.
• Experience working in a physician office.

Responsibilities

• Support CKD/ESKD patients with care coordination and resource access.
• Work from physician offices and perform in-home visits.
• Serve as primary liaison for provider practices and build trust.
• Document all activities in the care coordination platform.
• Educate practices about the Somatus program and optimize workflows.
• Facilitate transitions of care and schedule follow-up appointments.
