Job summary

You will perform face-to-face assessments and lead discharge planning, identify gaps in care, collaborate with providers, and educate patients to reduce readmissions. The role offers a hybrid model with in-person facility work and remote coordination.

Qualifications

• Current, unrestricted RN license in North Carolina.
• Minimum 3 years acute care nursing experience.
• Experience with discharge planning or care coordination.
• Experience with Microsoft Office and EMR systems.
• Valid driver’s license and reliable vehicle.

Responsibilities

• Conduct comprehensive face-to-face assessments for newly admitted members at acute care facilities.
• Lead discharge planning from admission through post-discharge and coordinate follow-up visits.
• Identify care gaps and barriers; develop strategies to address them.
• Collaborate with providers, case managers, and healthcare professionals for timely, coordinated care.
• Educate members on diagnoses, medications, and preventive measures to reduce readmission risk.
• Complete documentation accurately and promptly.
• Maintain infection control, safety, and patient privacy standards.
