Humana seeks an Onsite Care Coordination Nurse to support members during critical moments in their care journey, partnering with acute care teams to ensure safe transitions from hospital to home.

The role combines face-to-face assessments with remote documentation in a hybrid work model, requiring strong discharge planning, care coordination, and collaboration across providers. Registered Nurse license in SC is required; travel within 30-60 minutes of Greenwood, SC.

Qualifications

• Current, unrestricted Registered Nurse (RN) license in the state of South Carolina.
• Minimum of 3 years recent acute care nursing experience.
• Experience with discharge planning or care coordination in hospital or health plan settings.

Responsibilities

• Conduct comprehensive, face-to-face assessments of newly admitted members in acute care facilities.
• Support and educate members on discharge planning from admission through post-discharge.
• Identify gaps in care and barriers to discharge; develop strategies to address them.
• Collaborate with providers and care teams to ensure timely, coordinated care and effective discharge planning.
• Provide education on diagnoses, medications, and preventive measures.
