Job summary

Humana is seeking an Onsite Care Coordination Nurse to support members during critical moments in their care journey. You will conduct face-to-face assessments at CAMC General Hospital and lead discharge planning with care teams in a hybrid work setting.

You must be a WV-licensed RN with at least 3 years of acute care experience and proficient in MS Office and EMR systems. Travel and remote documentation vary by leader’s discretion.

Qualifications

• Current, unrestricted RN license in West Virginia.
• Minimum 3 years acute care nursing experience.
• Experience with discharge planning or care coordination.
• Experience in MS Office and electronic medical records (EMR).
• Valid driver’s license and personal vehicle insurance.

Responsibilities

• Conduct face-to-face assessments for newly admitted members at acute care facilities.
• Educate members and families on discharge planning from admission through post-discharge.
• Identify gaps in care and develop strategies to address barriers to discharge.
• Collaborate with providers, case managers, and healthcare professionals for coordinated care.
• Provide education on diagnoses, medications and preventive measures.
• Complete all required documentation accurately and promptly.
• Maintain infection control, safety and patient privacy standards.
